College of Dentistry Offboarding process is designed to facilitate the termination of services associated with the exiting user/member of the College.
Other Service Names
Termination, Deactivation, Retiring, Access removal.
To offboard a team or group of more than 5 members, please mention the names and NetID of the users to offboard in the "Additional Details(if any)" section in the Request Service form with the same required fields as in the requirements below.
Features
This form is to be completed and submitted by the Business Manager or direct supervisor of the exiting user. Once the form is filled out, the following services associated with the exiting employee will be deactivated:

Keys and building access

IT equipment handover

IT system access
Please remember to return all devices assigned to the exiting employee, including desktops, laptops, monitors, docking stations, and any other relevant equipment
Requirements
The requester should have the following details about the user who needs to be removed from the system:
- NetID
- Full Name
- Termination Action: Immediate (Urgent) / End of Business Day (Today) / On Termination Date
- Termination Date
