Description	This service is used to submit a request for HFS data from the OMI Data Team. You must have a valid Exhibit E on file with OMI for any request to be processed. See Attachment section to the right for blank Exhibit E form.

Who can use the service	OMI and HFS staff, including contractors, vendors, and partners, as well as current and former University of Illinois students and faculty.
