This form will need to be completed and returned prior to receiving alumni information from the Education Advancement Office by way of the University of Illinois Foundation. Team members from the Education Advancement Office will direct you when this form will need to be completed; please only complete this form if directed to do so. Form will need to be signed, saved as “UIF Conf Agreement 2020 – [last name]” and returned as an attachment to advancement@education.illinois.edu.
