How do I get authorization to put an application in Azure?

This article walks the end user through the process of obtaining permission to have an application hosted in the cloud via an application request form found at: http://cloudriskform.uic.edu

In order to host an application in the cloud, you must obtain permission from the cloud council.  In order to submit your application for consideration, you must:

1. Log into http://cloudriskform.uic.edu

2. Fill out any pertinent information and click submit at the bottom of the screen.
NOTE: As part of the approval process, there's a spreadsheet that's linked to the form that needs to be downloaded, filled out, and attached that helps determine the potential risks of hosting your application in the cloud.

3. Submitting the form will automatically notify the cloud council via Teams of your request and email them to ensure the matter is brought to their attention.  When they either unanimously approve your request or a single member of the Cloud Council rejects your request, you will be notified of the outcome via email.  Should your request be rejected, you may edit and resubmit your original request via a link in the email or submit an entirely new request.
